Potential Services
Funding for:

• In-home respite care (respite care is the hiring of a temporary/substitue caregiver in order to provide periods of rest and/or relief for a primary caregiver, be they family member, friend or agency personnel.)




• Adult Day Center, Assisted Living Facility or Nursing Home respite care.




• Care related educational opportunities for the care giver (ie: classes, seminars, workshops, support groups,...).




• If not receiving Medicaid benefits, payment for a Case Manager to assist clients and their families in accessing appropriate resources (private pay case management).




• Money towards equipment to enhance the caregiver's ability to provide care (ie: lift chairs, tub benches, handheld showers,...).

The Process
The person providing care (caregiver) and/or the person needing care (client) determine(s) the type of service needed and selects the service provider.
Northwest Colorado Options for Long Term Care does not employ, license, certify or otherwise control providers and is not responsible for quality of care. All arrangements for care and services are made by the client or someone the client has chosen to act on their behalf.
Northwest Colorado Options for Long Term care acts as a "flow through" agency, matching funds with caregiver needs.
Funding cannot exceed $100.00 per 24 hours of care and will be no more than $500.00 per year per caregiver/client partnership.

To Apply
You must apply in advance to be considered for funding provided through the Caregiver Support Program.
If you or the person for whom you are caring are currently receiving services from Northwest Colorado Options for Long term Care, please call your Case Manager before applying for this program. If not follow the steps below:

1. Call Northwest Colorado Options for Long Term Care toll free at 1-877-963-8731 to request a Caregiver Support Program application.




2. Complete the application.
