helps people with developmental disabilities get the information they need to understand their options and make informed choices to reach individual and family goals.  Individuals and their families are supported in accessing community resources, making transitions, and selecting appropriate services. Service Coordination also assists with exploring options such as Social Security, other county benefits, L.E.A.P., or food stamps.

Service Coordination
